Family Medical Release
For any youth attending a special event or trip, a medical release form is required to participate.
You only need to complete one form per household. Have your insurance card ready and the tetanus vaccine information for each youth. This release covers your youth from August 2023 through August 2024. Login to auto-fill some form fields.
